More than 1,000 tourists were being evacuated from Indonesia's tiny Gili islands.

According to Caruso, even though the Lombok quake occurred on land, it struck right along the coastline, where subsequent landslides into the sea could trigger tsunamis.

"First we used our hands to clear the debris, then hammers, chisels and machines to slowly remove the pieces", said Mr Marcos Eric, a volunteer rescue worker from a mining company, after an ambulance took the woman away.

Indonesia's National Disaster Mitigation Agency spokesman Sutopo Purwo Nugroho said on Wednesday more than 156,003 refugees had been created by the quake, a day after admitting assistance had not reached all because of road closures, and markets and shops standing empty.

"We lack the infrastructure to perform operations because (they) need to be performed in a sterile place", Wibowo told AFP. Food, medical supplies, tarpaulins and clothes are also urgently needed, it said. "Clean drinking water is scarce due to the extremely dry weather". "We are doing our best so we can fly out as many as possible".

The island was further rattled by a magnitude 5.2 quake.

Aram.the death toll has risen from Sunday and officials fear the number could increase even further.

The epicentre of the quake was in northern Lombok, a more residential, less developed part of the island.

"There are reports. that there are people buried alive, it is a critical time for immediate evacuation", he added, without giving details.

The quake triggered a tsunami warning, and frightened people rushed from their homes to higher ground, particularly in North Lombok and Mataram, the capital of West Nusa Tenggara province.

The Indonesian Red Cross said it had set up 10 mobile clinics in the north of the island.

"Teams are speaking of coming across ghost towns, villages that have essentially been abandoned", Matthew Cochrane of the International Federation of Red Cross and Red Crescent Societies said in Geneva on Tuesday.

It's a much needed moment of respite for the family - Montgomery-Lay says they've been awake for nearly 60 hours since being evacuated from their hotel room Sunday night.

Like Bali, Lombok is known for its pristine beaches and mountains. Hotels and other buildings in both locations are not allowed to exceed the height of coconut trees.

Indonesia sits on the Pacific Ring of Fire and is regularly hit by earthquakes. In December 2004, a massive magnitude 9.1 quake off Sumatra triggered a tsunami that killed 230,000 people in a dozen countries.

The Bali and Lombok airports have remained open.
